A project whose goal was the implementation of a nursing model focused on patient and nursing at the Clínica Universitaria Bolivariana's (CUB) gyneco-obstetrics service. The model was developed phase by phase, under a quality and quantitative methodology. An initial diagnosis of the professional care was done through a descriptive study: Bases, systematization, documentation, continuity, individuality, integrality, closeness, interdisciplinary, coordination, differentiation, visibility and quality. We concluded that there are fields in caring that require intervention; systematization, documentation and visibility among others. During the documentation phase, the principles of several theoretics of nursing were studied in order to determine the degree of applicability in the clinical field, aswell as a series of experiences of theoretical-practical integration. Virginia Henderson's theoretical fundamentals for guiding care were chosen because the group identifies it self with her philosophy and rules, focused on satisfying the person's needs among other reasons. During the operational designing phase the need to work according to the Nursing process was established; the most frequent nursing diagnosis was defined and made operational per patient group and per set of needs; the relevant clinical records and educational brochures were designed focused on needs. During the implementation phase,the defined processes were carried out. Currently, follow up and maintenance activities focused on training and research are in the process of being developed. The impact will be measured once the model is consolidated. We expect this project to be an example to develop similar experiences in other clinical services
Proyecto que tuvo como objetivo aplicar en el servicio hospitalario de Ginecobstetricia de la Clínica Universitaria Bolivariana (CUB) un modelo de cuidado centrado en el paciente y la enfermería. Se desarrolló por fases, bajo una metodología cualicuantitativa. Mediante un estudio descriptivo se hizo un diagnóstico inicial del cuidado profesional: fundamentación, sistematización, documentación, continuidad, individualidad, integralidad, cercanía, interdisciplinariedad, coordinación, diferenciación, visibilidad y calidad. Se concluyó que existen campos del cuidado que necesitan intervenirse; la sistematización, cercanía, documentación y visibilidad, son algunos de ellos. En la fase de documentación se estudiaron los principios de varias teóricas de enfermería para determinar su grado de aplicabilidad en el ámbito clínico, así mismo, una serie de experiencias de integración teoría-práctica. Se eligieron los fundamentos teóricos de Virginia Henderson para orientar el cuidado, porque el grupo se identificó con su filosofía y conceptos, centrados en la satisfacción de las necesidades de la persona, entre otros motivos. Durante la fase de diseño operativo se estableció la necesidad de trabajar en concordancia con el proceso de Enfermería; se definieron y operacionalizaron los diagnósticos de enfermería más frecuentes, por grupo de pacientes y por necesidades; se diseñaron los registros clínicos respectivos y plegables educativos centrados en necesidades. Durante la implementación se llevaron a cabo los procesos definidos. Actualmente se desarrollan actividades de seguimiento y mantenimiento, centradas en la capacitación e investigación. Se medirá el impacto una vez se consolide el modelo. Se espera que este proyecto sea un ejemplo para desarrollar experiencias similares en otros servicios clínicos
