The project aims to make significant improvements to the Chilean health system. Despite the relevance of the proposed changes, it does not eliminate the segmentation of the insurance system, which complicates its implementation. The law proposes to eliminate pre-existing conditions and risk tables, as well as to establish a Common Health Plan with flat rates, seeking to guarantee equitable and universal access. The project also includes the creation of a mandatory reinsurance system to mitigate the risks of adverse selection. However, this measure appears insufficient to ensure the financial stability of the ISAPRE system. The article highlights the need to establish an inter-ISAPRE compensation fund and measures to regulate mobility between FONASA and ISAPRE. Furthermore, the creation of a Common Plan, along with a reference fee for calculating coverage, presents an opportunity to enhance the model of care and quality of services. However, there is a significant risk if the approach taken is to contain costs by reducing prices
El proyecto de ley tiene como objetivo mejoras importantes del sistema de salud chileno. A pesar de la relevancia de los cambios planteados no termina con la segmentación del sistema de seguros, lo que complejiza su implementación. La ley propone eliminar las preexistencias y la tabla de factores, así como establecer un Plan Común de Salud con tarifas planas, buscando garantizar un acceso equitativo y universal. El proyecto también contempla la creación de un sistema de reaseguro obligatorio para mitigar los riesgos de selección adversa. Esta medida aparece como insuficiente para asegurar la estabilidad financiera del sistema ISAPRE. El artículo plantea la necesidad de establecer un fondo de compensación inter-ISAPRE y medidas para regular la movilidad entre FONASA y las ISAPRE. Por otra parte, la creación de un Plan Común, con un arancel referencial para el cálculo de las coberturas, genera una oportunidad para impulsar mejoras del modelo de atención y calidad de la atención, pero tiene un riesgo importante si se elige el camino de contener costos por la vía de reducir los precios
